What Is Refractive Lens Exchange?

Refractive Lens Exchange (RLE) — sometimes called clear lens exchange — replaces your eye's natural lens with a premium intraocular lens (IOL) to correct your vision and reduce or eliminate the need for glasses.

It uses the same refined technique as cataract surgery, but is performed for vision correction before a cataract has formed. For many patients over 45 — especially those with presbyopia or farsightedness who aren't ideal LASIK candidates — RLE offers a broad, lasting range of vision.

Because your natural lens is replaced, you'll also never develop a cataract in that eye in the future.

Why Patients Choose RLE

Freedom From Reading Glasses

Premium lenses can restore a range of vision — near, intermediate, and distance — addressing presbyopia that LASIK cannot.

Great for Farsightedness

An excellent option for higher farsighted prescriptions and patients who aren't ideal candidates for LASIK or EVO ICL.

You'll Never Get Cataracts

Because the natural lens is replaced with a premium IOL, you won't develop a cataract in that eye later in life.

Your Choice of Premium Lens

Choose from advanced lens options — including Vivity, TECNIS Odyssey, and PureSee — matched to your lifestyle.

Are You a Candidate for RLE?

RLE is often a great fit for adults in their mid-40s and older who want greater independence from glasses. You may be a candidate if you:

  • Are noticing presbyopia — needing reading glasses or bifocals
  • Are farsighted, especially with a higher prescription
  • Have been told you aren't an ideal LASIK or EVO ICL candidate
  • Want a lasting solution that also prevents future cataracts
  • Are motivated to reduce your dependence on glasses

Refractive Lens Exchange — Common Questions

What is Refractive Lens Exchange (RLE)?

Refractive Lens Exchange (RLE), also called clear lens exchange, replaces your eye's natural lens with a premium intraocular lens (IOL) to correct refractive error and reduce dependence on glasses. It uses the same technique as cataract surgery, but is performed for vision correction before a visually significant cataract has formed.

Who is a good candidate for RLE?

RLE is often an excellent option for adults in their mid-40s and older who are farsighted or experiencing presbyopia, want greater freedom from glasses, and may not be ideal candidates for LASIK or EVO ICL. Dr. Shah confirms candidacy with a comprehensive eye evaluation.

How is RLE different from cataract surgery?

The technique is essentially the same — the natural lens is replaced with a premium IOL. The difference is timing and purpose: cataract surgery removes a clouded lens and is medically necessary, while RLE is an elective, refractive procedure performed before a significant cataract develops. Because the lens is replaced, you won't develop a cataract in that eye later.

How is RLE different from LASIK?

LASIK reshapes the cornea, while RLE replaces the lens inside the eye. For patients over about 45 with presbyopia or higher farsightedness, RLE can deliver a broader range of vision than LASIK and prevents future cataracts. Dr. Shah will help you decide which procedure best fits your eyes and goals.

Is RLE covered by insurance?

RLE is an elective refractive procedure and is generally not covered by insurance or Medicare. The office reviews all costs, premium lens options, and financing during your consultation.
